Gliosarcoma is a rare high-grade mixed tumor that contains gliomatous and sarcomatous components and has a tendency to metastasize extracranially. We report a case of 39-year-old woman with a multiple intracranial gliosarcomas which mimic multiple brain metastases and metastasize to extracranial organs. Brain magnetic resonance image(MRI) revealed multiple intracranial ring-enhancing lesions. The pathologic diagnosis was gliosarcoma. We performed radiation therapy and chemotherapy. For nine months after treatment her symptoms were not worsened and her lesions were not aggravated. At thirteen months after first operation, she complained severe abdominal pain and follow-up positron emission tomographycomputed tomography(PET-CT) revealed multiple adrenal, peritoneal, hepatic, pancreatic, cardiac metastases. This report is first report of adrenal and peritoneal metastases of multiple intracranial gliosarcoma.
